Unlocking the power of people-driven care – Our annual report 2024–25

Welcome to our annual report for 2024–25. This year, our work has focused on listening to people across Derbyshire and making sure their voices are heard by those who can improve health and social care services.

From GP access, to hospital discharge, vaccinations, to pharmacy care - your feedback has shaped real change. Our vision remains the same – to put local people ‘centre stage’ so services are designed around the needs and experiences of our communities.

Highlights from the past year

  • 593 people shared their experiences of health and social care with us
  • 380 people came to us for trusted advice and information on services like GP appointments, NHS dentists, and social care
  • We published 15 reports to highlight where services are working well and where improvements are needed – including our most popular report on Pharmacy First, which raised awareness and improved communication across Derbyshire
  • Our 54 volunteers gave 2,892 hours of their time to help us reach communities, gather feedback, and support our work
  • We worked on major projects that led to real change, including improving hospital discharge, making vaccination information clearer, and influencing stop smoking services.
