Improving hospital discharge: New Care Transfer Hubs

The Joined-Up Care Derbyshire (JUCD) discharge from hospital improvement team has been speaking with partners and the public to understand the community's needs.

When someone leaves hospital but still needs extra support, intermediate care services play a vital role in helping them recover and increase their independence. These short-term services can help with:

  • Remaining in your own home when daily tasks become difficult
  • Returning home more quickly after a hospital stay
  • Recovering after an illness, fall, or operation
  • Avoiding unnecessary hospital admissions

Until now, referrals for intermediate care in Derbyshire were handled by different teams. To simplify and improve the process, the Joined-Up Care Derbyshire (JUCD) Discharge from Hospital Improvement Team has worked to introduced a new Care Transfer Hub (CTH).

What is the Care Transfer Hub?

The CTH will act as a single point of contact for intermediate care referrals. This means that people leaving hospital can get the right support when leaving hospital. With services being arranged by one team it will mean a more coordinated and efficient approach.

Phase 1 began on 7 January 2025. This phase is focusing on referrals for Derbyshire residents currently in a hospital bed outside of Derbyshire.

What’s Next?

Once they have reviewed the first phase, improvements will then be made based on feedback. The longer term plan is to work towards a service that processes referrals for all Derbyshire residents between 8:00am - 8:00pm 7 days per week.

As the hub develops, the team will grow to include people from the voluntary sector, housing, and transport.
