We want to hear from you on self-referral services

We have recently launched a new survey asking people about self-referral services in Derbyshire.

When we feel unwell, most of us would think about going to our GP, but there are lots of other services to help people stay well.  Some health and care services can be used without seeing a GP first. This is called self-referral.

We want to find out if people know about these services, if they have used them, and what their experience was like. We want to know how people would prefer to get information about these services. We would also like to hear from people who haven’t used these services so we can understand any barriers there are to access.
