We're excited to be hosting an event in September, focusing on hospital discharge in Derbyshire.

Why Attend?

Hospital discharge is an important part of patient care. Sometimes it can be an easy process of discharging people back to their homes. Sometimes however, discharging patients from hospital isn't as easy and can be challenging for both healthcare providers and patients.

This event will bring together healthcare professionals, patients, carers, and community members to explore the discharge process, identify gaps, and discuss strategies to help patients have a smooth transition from hospital to home.

What to Expect

  • Learn more about the different hospital discharge pathways in Derbyshire
  • Hearing from JUCD’s Discharge Improvement Team about their work, and their achievements so far
  • Working together to identify strengths, weaknesses, and opportunities for change in the current hospital discharge pathways
